A Comparison Study of Treatment Outcome in Panic Disorder with or without Agoraphobia

Abstract
The present study was performed to compare the treatment outcome of patients with panic disorder with or without agoraphobia, and to find the relationship between agoraphobia and the use of medication. The subjects were 57 patients who met DSM-IV criteria for panic disorder with agoraphobia and 18 patients who had panic disorder without agoraphobia. We assessed symptom severity and some cognitive behavioral indexes typically found in panic patients using Panic Disorder Severity Scale (PDSS), Clinical Global Impression (CGI), Spielberger State-Trait Anxiety Inventory (STAI), Beck Depression Inventory (BDI), The Revised Anxiety Sensitivity Index (ASI-R), Agoraphobic Cognition Questionnaire (ACQ), Body Sensation Questionnaire (BSQ), Automatic Thoughts Questionnaire-Negative (ATQ-N), Automatic Thoughts Questionnaire-Positive (ATQ-P). For statistical analysis, we performed paired t-test and repeated measures ANOVA test to compare the scores of scales between pre and post treatment in both groups.
Independent t-test was used to compare the scores at post treatment state. Also chi square-test was used to compare the use of medication between patients with agoraphobia and without agoraphobia during some specific phase of treatments. There were no significant differences in all scores of scales between both groups at post treatment.
Improvement of panic symptom severity was more prominent in patients with panic disorder with agoraphobia treated with CBT and pharmacotherapy and improvement was correlated with changes of anxiety sensitivity, agoraphobic cognition and positive automatic thoughts. The use of medication at pre treatment, at 1st week and 12th week showed no significant differences between two groups.
